How wide is the candidacy for a ChondroFiller injection?

For most adults with cartilage damage — whether a discrete focal lesion or widespread wear across a joint — the ChondroFiller injection pathway is broader than many patients expect.

There is no upper age limit. Patients in their sixties, seventies, and beyond are considered on the same clinical grounds as younger adults, and the injectable scaffold can be delivered across the entire articular surface in a single session, so there is no defect-size ceiling either. Eligibility extends beyond the knee: the injection is used across the hip, shoulder, ankle, elbow, wrist, thumb, and small hand joints, reflecting the scaffold's ability to conform to varied joint geometries. Both focal Grade III/IV cartilage defects and diffuse cartilage wear fall within scope.

Previous joint procedures are not a barrier. Prior arthroscopy, microfracture, meniscal repair, or ligament reconstruction do not disqualify a patient from the injection pathway and may, in fact, help confirm the extent of damage and establish suitability.

One distinction is worth noting early: the injectable form of ChondroFiller carries a broader indication than the surgical implant form, which is reserved for focal, contained defects with healthy surrounding cartilage borders. This article concerns the injection pathway — an ultrasound-guided outpatient procedure — not the surgical route.

Individual suitability, including joint-specific factors and any exclusion criteria, is confirmed through a structured clinical assessment.

Side effects to expect in the first 48–72 hours

Three symptoms are normal in the hours following a ChondroFiller injection, and recognising them makes them far easier to read.

Swelling typically peaks at around 24 hours. It reflects the scaffold gelling inside the joint and the synovial environment responding to intra-articular placement — not a sign of infection, irritation, or failure.

A dull, pressure-like ache accompanies the swelling in most patients. It is distinct from the sharp or throbbing quality of an infected joint; it feels more like a joint under temporary load than a joint under threat.

Mild stiffness is the third expected symptom, usually in proportion to the swelling and settling as it does.

All three should begin to ease after 48–72 hours. Direction of travel is the key marker: steady improvement is the expected arc. A plateau followed by gradual settling is still within the normal range; symptoms that worsen beyond the initial 48 hours are a prompt to contact the clinic.

One further reassurance: the scaffold's setting process is entirely internal. There is no sensation that tells a patient whether it has gelled correctly — the body's response, not a feeling of the scaffold working, is what patients notice. Clinics should provide clear written guidance on what is normal before the patient leaves, so that self-monitoring at home is grounded in accurate expectations rather than anxiety.

Warning signs that need prompt medical attention

Two categories of post-injection signal warrant prompt attention, and they present differently enough to be worth knowing before leaving the clinic.

Infection signals

An infected joint does not plateau — it worsens. Pain that continues to escalate beyond 72 hours rather than beginning to ease, fever, chills, or spreading redness and warmth around the joint are all triggers for same-day contact with the treating clinician. These feel different from the dull pressure-ache expected in the first day or two: the quality shifts — sharper, more insistent — and, critically, the trajectory is wrong rather than settling.

Allergic reaction

ChondroFiller's scaffold is derived from rat (murine) Type I collagen. The majority of patients have no sensitivity to this, but anyone with a known allergy to animal proteins or collagen-based materials should raise it at the pre-treatment assessment — before the injection, not afterwards. An allergic response looks distinct from localised joint discomfort: it may involve widespread skin changes such as urticaria or flushing, or systemic symptoms well beyond the treated joint. If any of these occur, the same same-day contact threshold applies.

Serious reactions following ChondroFiller injection are uncommon — the published post-market complaint rate is very low, a figure examined more fully in the evidence section below. Low rates do not reduce the importance of acting promptly if a red flag does appear.

Absolute contraindications — who cannot proceed

Four exclusions are absolute — no clinical judgement can modify them, and any single one rules out treatment until the circumstance changes or, in some cases, permanently.

Known hypersensitivity to murine proteins or Type I collagen. As established in the warning-signs section above, the scaffold is rat-derived. Anyone with a documented allergy to animal proteins or collagen-based materials must be identified and excluded before the injection; the risk cannot be managed by dose adjustment or antihistamine premedication.

Active local or systemic infection. Defer means cleared — not antibiotic cover, not infection suppressed to subclinical levels. Introducing a biological scaffold into a joint harbouring active infection would expose the implanted material directly to a bacterial environment. Treatment may be reconsidered once infection has fully resolved.

Active malignancy. Systemic disease of this kind places the patient outside the population for which any safety data exist. No exceptions apply.

Pregnancy or breastfeeding. No safety data for ChondroFiller exist in these populations. Treatment is deferred until after this period has passed.

The pre-treatment assessment is the structured point at which each of these criteria is reviewed — its purpose is precisely to ensure no exclusion is reached after the fact.

Relative contraindications and what the pre-treatment assessment looks at

Unlike the absolute exclusions already covered, the factors below call for clinical judgement rather than automatic refusal — some patients in these categories will proceed, others will not, and the pre-treatment consultation is where that determination is made.

Bleeding disorders and anticoagulation. Uncontrolled bleeding disorders and heavy anticoagulant use are not automatic stops. Each case is reviewed individually, weighing the coagulation picture against the demands of an intra-articular injection. Patients on therapeutic anticoagulation should discuss their current regimen with the treating clinician rather than assuming they are ineligible.

Severe immunosuppression. ChondroFiller's mechanism depends on recruiting the patient's own progenitor cells into the scaffold after it gels in the defect. Severe immunosuppression may blunt that cellular response and limit how effectively repair proceeds. This is assessed case by case; mild or well-controlled immunosuppression is a different picture from profound suppression.

Structural joint factors. Uncorrected malalignment greater than 5 degrees, significant joint instability, and major meniscal deficit each undermine the mechanical environment the scaffold needs to integrate. These are not disqualifiers — they are correctable preconditions. Malalignment may be addressed before or alongside the ChondroFiller injection; instability and meniscal pathology follow the same principle. Treating into a mechanically compromised joint bypasses the structural problem and risks poor scaffold integration regardless of product quality.

The pre-treatment assessment maps these variables together, rather than evaluating each in isolation, to arrive at a realistic treatment plan.

The safety evidence — what the data shows and what it cannot yet tell us

The headline figures from ChondroFiller's manufacturer Clinical Evaluation Report (CER Version 09, April 2025) are striking: across more than 19,000 treated cases compiled since 2013, zero serious adverse device effects (SADEs) were recorded, and the overall complaint rate sits at approximately 0.06% — fewer than one in every 1,500 treatments.

The caveats are real but finite. The entire dataset is manufacturer-sponsored, drawn from Meidrix Biomedicals GmbH's post-market surveillance, and reflects carefully selected patient populations. No blinded randomised controlled trial has been published. This limits the strength of inference that can be drawn — but it does not invalidate the figures.

External corroboration comes from Matta et al. (2025), a prospective peer-reviewed study published on PubMed Central, which examined ChondroFiller in 25 wrist cartilage-defect patients and found no significant difference in complications versus matched controls. The cohort is small; the direction of finding is consistent with the manufacturer data.

Supporting signals include MOCART scores of 81.6–84.3 in European studies — indicating that over 80% of treated defects were filling and integrating with surrounding cartilage — and a reoperation rate of 3–8%, compared with up to 41% for microfracture and up to 37% for ACI or MACI procedures.

What remains genuinely absent from the literature is long-term data from blinded, independent, multi-centre trials. That evidence has not yet been published.
