What physio can realistically achieve — and where it hits a wall
After several months of physiotherapy following an ACL injury, many patients arrive at the same question: is this still working, or is it time to consider something more? The honest answer is that there is no validated minimum duration that applies universally — the decision turns on lesion grade, defect size, the stability of the knee, and how symptoms are tracking over time.
Within those parameters, physio has a clear and legitimate role. The goals are conditioning, reducing high-impact joint loading, and establishing injury-prevention movement patterns — not regenerating lost cartilage. For low-grade lesions in a mechanically stable knee, this approach can meaningfully slow the degenerative process and keep patients functioning at a high level.
The wall appears under two conditions. First, Grade 3–4 lesions — where more than half the cartilage depth has been lost, or the subchondral bone is exposed — are unlikely to stabilise through load management alone. Second, and critically for this cohort, an ACL-deficient knee with ongoing instability can actively accelerate cartilage wear during a physio-only phase, meaning the conservative programme may be working against itself.
It is also worth stating that surgery does not guarantee a return to pre-injury function. That cuts in both directions: prolonged delay is not without cost, but neither is premature escalation. The sections below map the specific signals that shift the balance.
Why ACL rupture puts cartilage at extra risk
Half of all ACL ruptures arrive with unwanted company. At the moment the ligament tears — typically a rapid deceleration or twist — the same forces that sever the ACL also drive cartilage surfaces together with sufficient energy to create immediate chondral damage. Slauterbeck et al., reporting in the Journal of Bone and Joint Surgery (2009), confirmed that these lesions are not confined to one side of the joint: cartilage damage spans both the medial and lateral compartments, reflecting the multidirectional forces at work during the injury event itself.
That immediate damage sets a difficult context for recovery. Articular cartilage has no direct blood supply and, once injured, very limited capacity to repair itself — defects exceeding roughly one centimetre in diameter are recognised to worsen progressively over time. In an isolated, stable knee, a carefully managed conservative programme at least provides a relatively controlled loading environment.
An ACL-deficient knee does not offer that stability. Each episode of giving way applies abnormal shear forces to a cartilage surface that is already compromised. This ongoing mechanical instability acts as an independent driver of cartilage wear — separate from and in addition to the original injury. More severe ACL ruptures associated with significant co-injury carry a higher risk of accelerated degenerative progression. That is why the threshold for specialist review tends to be lower in high-grade combined ligament-and-cartilage injuries than in an isolated chondral lesion sitting within a mechanically stable joint: the conservative window, if it exists at all, is almost certainly narrower.

Lesion grade and size: the two numbers that shape your timeline
Two numbers tend to define how urgently the conservative-first window needs to close: cartilage grade and defect size.
Cartilage grade
The ICRS grading system runs from 1 to 4 based on how deep the damage extends. Grade 1–2 lesions involve less than half the cartilage depth; Grade 3–4 lesions extend beyond 50% or expose the subchondral bone beneath. For Grade 1–2 findings, supervised physiotherapy combined with active monitoring is generally a reasonable primary strategy — the structural loss is partial, and conditioning plus load management may slow further deterioration meaningfully. Grade 3–4 lesions occupy different territory: the structural integrity of the cartilage column is substantially compromised, and specialist review is warranted even if physiotherapy continues in parallel. Conservative management may still play a role, but it is unlikely to be definitive on its own.
Defect size
Size matters independently of grade. Lesions under roughly 2–4 cm² remain within the range where repair techniques such as microfracture or osteochondral autograft (mosaicplasty) are viable options, should the conservative phase prove insufficient. For defects at or above 3 cm², the SUMMIT trial provided direct comparative evidence: at both two and five years, MACI — matrix-induced autologous chondrocyte implantation — produced better KOOS pain and function scores than microfracture in this size range.
What these numbers actually mean for you
Neither grade nor size operates as a hard trigger. They are important inputs into a specialist conversation, not verdicts. What they do determine is how much time the conservative window realistically has. How to judge whether physio is working — or whether the window is closing

For post-ACL cartilage patients specifically, no clinical trial has established a minimum treatment duration (three months, six months, or otherwise) after which physio should be deemed to have had a fair trial. That absence is not a gap in care; it reflects that the meaningful signals are biological and functional, not calendrical.
The clearest green light to continue is a trajectory of improvement: pain reducing, range of motion and strength recovering, and imaging stable on sequential MRI. When all three are moving in the right direction, extending the conservative programme is well-supported.
The picture looks different when progress plateaus after a structured, supervised trial — or when symptoms worsen. In those circumstances the appropriate response is prompt specialist reassessment, not a longer wait. Delay, as established by the biology of avascular cartilage, allows a manageable defect to become a more complex one.
Certain symptoms warrant specialist review regardless of how recently physio began. Locking, catching, or giving way are mechanical red flags that suggest the lesion may be generating loose bodies or causing impingement — problems that conditioning alone cannot address.

Injectable support during the conservative phase
Between physiotherapy and surgical repair sits a middle tier that is often underused: injectable treatments that can support or extend the conservative phase without committing to theatre. Two options are relevant here, and they work quite differently.
PRP (platelet-rich plasma) is drawn from the patient's own blood, concentrated, and injected into the joint to modulate the local environment — reducing inflammatory signalling and supporting the tissue conditions around a defect. The evidence for PRP in knee cartilage management is reasonably established in broader chondral populations, but evidence specifically in the post-ACL cartilage cohort remains limited. Its role is symptom and environment support, not structural regeneration of the defect itself.

- A defect of 2 cm² or more on imaging, particularly where the patient is young and active and cartilage preservation carries significant long-term stakes.
- Ongoing concern that ACL-related instability may be actively undermining the conservative programme rather than running alongside it.
